This chat is guest hosted by @NHS_ICB

Monitoring and managing blood pressure is one of those fundamental nursing activities that we undertake regularly for the people we care for. As nurses and healthcare professionals, we know that blood pressure is hugely important and high blood pressure can have far reaching implications. Figures show that “Over 5 million people are unaware they have high blood pressure yet it affects more than 1 in 4 adults and is one of the biggest risk factors for premature death and disability in England.” (Public Health England 2014)

The recent publication Tackling high blood pressure: From evidence into action (Public Health England 2014) underlines the importance of leadership, collaboration and improvement across all areas of blood pressure pathways but specifically identifies three main areas:

Prevention – Detection – Management

We have adapted these areas into the graphic below:

This #WeNurses discussion welcomes all members of the multidisciplinary team, from Healthcare Assistants, Doctors and Pharmacists to all areas of nursing as each have a vital role in high blood pressure prevention, detection and management. We aim to explore the following questions:

•Why does high blood pressure matter?

•What can we do to prevent high blood pressure?

•What can we do to detect high blood pressure?

•What can we do to best manage patients with hypertension?

•How can we work as part of a Multi Disciplinary Team to prevent, detect and manage high blood pressure?

•How can we engage patients in their health – so they care about their blood pressure?

•What changes can you make in your role, to better prevent, detect and manage high blood pressure?

The discussion then moved on to prevention of high blood pressure:

Detection of high blood pressure was also explored:

Management of high blood pressure was a theme that was threaded throughout the discussion:

Making every contact count was also a key theme:

This discussion contained so much information, links and resources that it really is recommended that you read the transcript - however many left the chat temporarily to do this shared test from NHS Choices and reported good scores back... why not give it a try!
