More and more Advanced Practitioner roles are being created throughout the Health Care Service.

This is being driven by a number of factors:


·       Changes in medical training.

·       A realisation that there needs to be more consistency in the services provided.

·       Difficulty in recruiting Doctors to posts in various specialities.

·       An availability of clinicians who want to expand what they do.


Who is defining what an Advanced Practitioner is- the RCN has provided some guidance which is provided as a link, but there is also much debate about what they should and shouldn’t be doing- The Role of Advanced Nurse Practitioners (Nursing in Practice), Use advanced nurse practitioners to fill gap in GP workforce (Independent Nurse) but there is possibly still some confusion about their roles and the differences between them and other health care providers- Differences between clinical specialist and advanced practitioner clinical practice, leadership, and research roles, responsibilities, and perceived outcomes (the SCAPE study).

But the development of this new role leads to many more questions than answers sometimes and our aim in this chat is to generate a debate around some of these questions and to see if we can come to any conclusions.

Just under a hundred participants joined our advanced practice tweetchat, with a good mix of roles and professions. It was great to have academic,clinical academic, clinical and non-clinical tweeters join us! We had a great discussion from a really chatty group, so much so we didn’t get to ask all of our questions!

We started the discussion around how much ‘nursing’ experience you should have before going into advanced practice? A number of tweets highlighted the importance of competence, capability and experience rather than a time frame although @amita_sherait suggested at least 5 years @JuneinHE tweeted less about time served, more about knowledge and application of knowledge in practice.

The next question posed was to define advanced practice. Tweets suggested @laura52153139 wisdom, courage and leadership skills @diabetespc knowledge and skill to be able to work independently @LynneBowers2 knowledge skills and behaviours beyond specialist @amita_sherait AP is someone who could put theory into practice at advanced level and make autonomous decisions.

We asked our tweeters what is difference between advanced and specialist practice @CraigBradleyF1 specialist practice is advising on and supporting decision making, advanced practice is making the decision, for me at Least! Lovely tweet from @hospitalcaterer ALL you do is special I saw this from a personal perspective. Reminding us the patient is at the heart of all practice whatever the title/role 

The importance of research and academic level of study got a few mentions J @CraigBradleyF1 Part of advanced practice is advancing that practice and therefore research contributions play an important part @squeekyspeaks what ever the academic level so much about the application @baconandbraces MSC level education needs to be focused on clinical care and skills to advocate for autonomy rather than theory @HeatherBain9 needs to be at postgraduate level to represent the level of critical thinking and complexity of role @chuckyhorton Adv prac = Academic, clinical knowledge,clinical skills, leadership and management decision making & most important love to care! @astaines ..Doing research matters, but so does integrated audited reflective and evidenced based practice

We asked who defines advanced practice? universities, governing bodies,professional bodies? @GCUDNS NHS boards investing in the development of ANP forthe long term @Elizsummary Better if advance practice has one #regulated title @brianlynn29my first thought is professional bodies @xray_nick National bench marking / accreditation by prof bodies ensures AP recognized.

Final thoughts THE FUTURE 4 AP’sJ @amita_sherait Future…Way to go…its going to reform NHS  @CraigBradleyF1 Big and Bright future, so many positive impacts on patients outcomes &enriching the evidence base @Lynnbowers2 reminded us that at present not consistent and asks is that the problem? Clarity and assurance for the public @alisonleary1 too many titles, no clarity of role, misconceptions (i.e. Drsubstitution instead of added value) @matthewvanloo a standardized career development pathway supported by employers and to education @Jemmaowen agree need standardized pathway and educational route nationally
