For this chat we are lucky to have Holly Philips, a third year learning disability nursing student and Student Nursing Times Award 2016 finalist, join us again to guest host a chat on a topic that Holly has been researching as part of her placement.  Holly is currently studying at the University of Cumbria where she actively participates in student peer support, is a student quality ambassador and campaigns for Changing Places.

''As part of my placement I have been set a piece of work looking at the use of selective serotonin re-uptake inhibitors(SSRIs) in children with learning disabilities for the reduction of symptoms from Autistic Spectrum Disorder (ASD).

NICE, (2013) says that 50% of children with a learning disability are diagnosed with ASD and is generally more commonly diagnosed within boys.  According to NICE (2013) ASD is often accompanied by another psychiatric disorder requiring treatment, such as attention deficit hyperactivity disorder, this can be present in up to 70% of cases but can often go unrecognised.

My research so far, which has included many a journal hunt, including a read of a Cochrane Review (link below) which included 5 studies of children taking SSRIs, each leading to a conclusion to say that there is little to no evidence that use of SSRIs has any effect for these purposes.

Within placement however I have come to learn of cases to show the contrary. For example a young girl who has been intolerant of clothes, so will not dress, and is unable to leave the house presenting with very aggressive behaviour. After an unsuccessful course of experimenting with desensitisation/social stories etc. she has recently been trialled on fluoxetine and is now able to tolerate clothing to leave the house and go to school, shopping and out to play.

NICE (2013) indeed say that psycho-therapeutic methods should be used prior to considering medication, but this should be done on a case by case basis, where there may be decisions made to prescribe medication for ASD symptoms the beneficence and side-effects to the person must be thoroughly considered.

So considering this, there seems to be very few cases or pieces of research presenting some of these case by case situations

So my questions are these….

  2. What treatmentpathways do you use for helping the symptoms of ASD in children?

  3. Does it differ if youhave someone with or without a LD?

  4. What psychotherapyapproaches have you used for children with ASD & LD?

There would appear that those who have joined in the chat had, unsurprisingly, mixed experiences. This would most likely have to do with the services they were in or areas they were working. It would however appear that generally there was less of an emphasis to prescribe children with medication that there is amongst adults; with the speculation that it is more of a ‘goto’ approach in adults than in adolescents, but could this be as we grow older/bigger are our physical/behavioral symptoms harder to manage?


Throughout discussion the difference in school vs home demands appeared to be different too. This seems to be related to the responsibility of giving medication and therefore possibly the responsibility of that administration, whichever way round it is being requested. I feel though it is important to note at this point that the issue of waiting lists was brought up, as was the long waits families often face in services. Experiences of parents wanting medication was often referred to as a last resort and because they were “at the end of their tether” it is then perhaps as a result of those waiting lists parents/schools wish to explore the medication route more keenly? Had the situation escalated so much that medication could be seen as an ‘easy’ option or ‘quick’ fix?

A number of strategies were listed to try and support people with symptoms without the use of medications, social stories, communication strategies, positive behavior support, structured and regular respite, mindfulness, sensory profiles, Speech and language and Occupational Health input. As well as ensuring that the environmental and structure avenues are explored, such as having strong routines in place and ‘safe spaces’ for a child to be able to go to experience a calming and relaxing atmosphere.

In an ideal world each of us, in whatever service we work in, wish to be able to refer/triage/set pathway etc. as soon as possible, however within the reality of the high demands to services this is becoming more and more difficult every day. The overall conclusion to the discussion led to us recognising the benefits as well as factors to be wary of when considering the use of medication but first and foremost that we must be person centered and be in line with NICE (2013) to consider our strategies on a case by case basis, encouraging the consistency within the strategies which are best set for that child and family.
