In the time it took you to start reading this pre-chat, someone in the world had a stroke. During their lives, 1 in 6 people will have a stroke and 1 in 7 people that have a stroke die within 30 days. For those that survive, a stroke can be a life-changing event. But what do we really know about it?

A stroke is defined as a rapid onset of focal neurological deficit lasting more than 24 hours, with no apparent cause other than disruption of the blood supply to the brain (World Health Organisation 1989). It can either be ischaemic (caused by a blockage in an artery of the brain) or haemorrhagic (caused by an artery that bursts in the brain). Emergency treatments for acute ischaemic stroke such as thrombolysis (clot-busting treatment) and most recently thrombectomy (mechanical clot removal) have showed to reduce morbidity and mortality. They are examples of life-saving treatments and also responsible for the reason why stroke has become a much more attractive area to work as a nurse: it is impossible not to feel that you are able to make a difference!

Since 2007, national drivers have put stroke at the centre of healthcare and the NHS. In London, specifically, the development of the London stroke model with hyper-acute stroke units, acute stroke units and services for Early support discharge and/or community rehab services have redesigned the way care is provided to people with a stroke but more is needed. Recently, the Stroke Association has put together a petition asking for a “New Era for stroke” that has had more than 55 000 signatures (https://www.stroke.org.uk/what-we-do/our-campaigns/new-era-stroke).

But how can we recognise a stroke? How can we prevent a stroke from happening and how can we save those that present with symptoms of a stroke? This twitter chat aims to answer all these questions and others you may have!

I’m Ismalia de Sousa, a EU trained nurse from Portugal and a clinical nurse specialist in stroke at Imperial College Healthcare NHS Trust. For the past 7 years I’ve been a registered nurse for the past, working in stroke care and having published in this area. In my current role, I’m a change agent/team leader and I’m involved in developing the emergency stroke pathway in the trust, most recently the thrombectomy pathway. In the last 3 years, I have also redesigned how secondary stroke prevention education is provided to patients and families. Asa CNS in stroke, I’m also involved in the educational development of nurses and practice improvement across the stroke services. In 2015, I became a member of the RCN Neuroscience Forum Steering Committee and this year,at the RCN Congress, together with the RCN Neuroscience Forum SteeringCommittee we held a fringe event entitled “How to save a stroke patient in 1hour?” (https://www2.rcn.org.uk/newsevents/congress/2016/events/monday/how-to-save-a-stroke-patient-in-one-hour)to raise the profile of stroke nurses and stroke care in the UK.


Dr SueWoodward worked clinically within the field of neurosciences nursing almost exclusively since she qualified as a registered nurse in 1988 and has worked in a number of regional neuroscience units around London. Sue became a full-time lecturer at King's College London in 1999. She completed a four-year term as head of department for specialist and palliative care and completed her PhD studies in 2011. Sue has developed a MSc pathway in Advanced Practice - Neuroscience Care within the School. Sue has also co-edited the “Oxford Handbook of Neuroscience Nursing” (2009) and "Neuroscience Nursing- Evidence-based Practice" (2011), the only two neuroscience nursing texts published in the UK. Sue’s current research focuses on bowel management in inflammatory bowel disease and Multiple Sclerosis. Sue has particular interests and expertise in bladder and bowel care within neuroscience, management of patients with long-term neurological conditions and clinical education. She is Chair of the Royal College of Nursing Neurosciences Forum committee, founding editor and Editor-in-Chief of the British Journal of Neuroscience Nursing, Commissioning Editor of the British Journal of Nursing and was a member of the NICE Guideline Development Group for urinary incontinence in neurological disorders.

The RCN Neuroscience Forum Steering Committee represents RCN members working in neurosciences by promoting its value. It also promotes best practice and influences change. As “CVA” (cerebrovascular accident) is a term outdated and the term “stroke” should be use instead. Some people may also call it a “brain attack” but a stroke is a clinical syndrome in which focal (or global in case of coma) disturbance of cerebral function occurs suddenly; every 5 seconds someone will die of stroke in the world. The median age of stroke survivors is 77 years old but 25% of all people that have had a stroke are less than 65 years old. Interesting enough, during this twitterchat, the Stroke Association from the North West region noted that referrals for ongoing support for people younger than 65 years old represented 1/3 of all their referrals.

It is through the FAST (Face, Arm, Speech, Time) campaign (https://www.youtube.com/watch?v=vc9OF64H4sE) that we can recognise some signs and symptoms of stroke. Other symptoms may also include sudden onset of headache, sudden visual loss, etc.  Stroke is a medical emergency and “time is brain”. Like @AgencyNurse has said “it is important to deal with it quickly as the as the effects are devastating to people and families”, but also to their relationships, work and not forgetting the psychological effects, many times hidden.  

But why do strokes occur? Between younger and older populations, the reasons may differ but @SueWoodward_KCL mentioned the biggest modifiable risk factor, which “is hypertension, others include smoking, obesity, sedentary lifestyle etc”. However, in some cases we never understand the reasons for why the stroke occurred.


As the population is ageing we may be seeing more people having strokes but we are better at recognising, treating and preventing hospital admissions (an example being the development of Transient ischaemic attack clinics). We are also better in rehabilitation and in promoting self-management and neuroplasticity. And even in more rural and remote areas innovative models have been developed.
