Independent advocacy gives patients and carers the support they need to exercise their rights, express their views and make informed choices in all aspects of their lives. It can help to place people at the centre of decision making and ensures that they retain control over their lives.

If a person wishes it, an advocate can represent their views and wishes and can challenge decisions, abuse and injustice on their behalf.  The advocate is there solely for the person, without seeking to advise or influence them, and with no conflict of interest.

The Older People’s Advocacy Alliance promotes advocacy for older people.  There are many issues that older people might want advocacy support to face, including but not limited to making decisions about health, care, finances and housing.  Advocacy can play a vital part in safeguarding older people from abuse. There are many reasons why an older person may feel they need this support.  Some do not have anyone in their lives to support them to explore their options and make informed choices- around 3.5 million people aged 65+ live alone and by 2030 there will be 2 million people over 65 without children. But advocacy is not just for the unbefriended; navigating complex health and care systems can be confusing, isolating and frightening for all of us.

This discussion aims to explore some of the following questions:

·      How do you advocate for your patients?  Are there limitations or restrictions to your advocacy role?

·      What does "independent advocacy" mean to you? 

·      Have you worked with a patient who has had an independent advocate? Or made a referral to an advocacy service?

·      Who can benefit from independent advocacy support?

·      How can advocacy complement the role of the clinician?

·      What barriers are there to referring into advocacy services?

Mike’s Story, recently released on OPAAL’s YouTube channel highlights some of the many issues advocates support older people affected by cancer with.  Mike talks about the impact of his cancer diagnosis, his recovery from alcoholism, mental health and how his life was taken over by the thought of cancer and how things changed for him when he was introduced to Bob, his volunteer peer advocate.

Meet David his family and advocates as he talks about the impact of his cancer diagnosis, his wish to remain at home for as long as possible, and how his advocate offered him support, increased confidence and companionship as his treatment plan has changed.

Background

We’re the Older People’s Advocacy Alliance (OPAAL UK) the only UK based national organisation supporting independent advocacy services for older people.  We believe older people have the right to independent advocacy to support them to exercise their rights, express their views and make informed choices in all aspects of their lives.

Since 2012 we’ve been delivering a cancer, older people and advocacy programme in partnership with Macmillan Cancer Support and we have been working closely alongside clinicians to increase their understanding of the role advocacy can play in promoting patient voice, how this complements their role and the benefits for patients and carers. 

We made a submission to We Nurses to start a conversation on social media with health care professionals about the role of the independent advocate in health and care settings, helping us reach more professionals. 

The chat

Some of the themes drawn out in the #wenurses twitter chat:

  • Certainly the chat confirmed our thoughts that there is a need for more awareness raising around the role of independent advocacy with health and care professionals
  • There seemed to be agreement that the aims advocates and nurses have are the same in regards to decision making and patient voice, some participants highlighted that their advocacy role is enshrined in their code of conduct but there was a broad spread recognition of the ‘independence’ of independent advocates who are structurally, financially and psychologically separate from health and care services, with participants talking about the barriers to their nursing advocacy role “conflict with team members” “sometimes speaking out can make HCPs unpopular with others” “nurses bound by policy and procedure”“bound by Professional Conduct.  One of the chat participants shared an interesting article on the barriers to advocacy in the nursing role -http://bmcnurs.biomedcentral.com/articles/10.1186/1472-6955-5-3
  • Health care professionals’ time was a key theme – we really liked this quote; “time at the time it is needed.”  This really chimes with advocates, as a participating advocacy provider said “we can support the person with non-clinical issues freeing the clinician's time”.  There was also discussion about support needed at pre and post op stages of care, again advocates have the gift of time, and the flexibility to support people in their homes and communities, outside of clinical settings
  • There was limited but positive discussion from HCPs who had worked alongside independent advocates, their experiences fit with what we know about how the advocacy role can complement the nursing role.  Recognition that advocates can bring in a different knowledge base (support outside the clinical, around benefits, housing etc),can “help speed up discharge planning, take so much pressure off ward staff”and “they help us reflect; improve our care, by working with YP and staff to create effective and understandable dialogue”. 
  • Finally, more questions were raised than answers in response to the question “what are the barriers that stop clinicians referring to independent advocacy” indicating that more work is needed to raise awareness – “Who pays for advocacy?” “Not knowing how or who?!”
