As a reaction to ageing populations with increasingly complex healthcare needs, healthcare systems have begun to shift the focus of care away from acute, reactionary care and move towards investing in health improvement strategies and preventative, community based care (Scottish Government, 2013). This developing approach to the management of long term conditions and end of life care isreferred to as advance or anticipatory care planning. Within the literature advance care planning tends to be associated with end of life care discussions and decisions (Boot, 2016),whereas anticipatory care planning tends to be associated with the management of long term conditions (Kennedy, et al., 2011). NHS Scotland does not differentiate between the two terms. This is because both describe a philosophy of care that aims to promote individualised care, enhanced communication and respect for the preferences of those whose lives are limited through disease or illness (Scottish Government, 2010). However, NHS England tends to refer to advance care planning (Department of Health, 2012).

Comprehensive advance/anticipatory care planning involves informal steps such as voluntary discussions with families and friends, and formal steps such as the appointment of Power of Attorney, and completion of a Will, Advance Directives and Do Not Attempt Cardiopulmonary Resuscitation(DNACPR) orders (Mullick, et al., 2013). It is recommended that this information is documented in a patient held advance/anticipatory careplan, however, there is currently no standardised approach to completing these plans.

The Nursing and Midwifery Council (NMC) Code (NMC, 2015) states that nurses should care for individuals without making assumptions and will recognise personal choice and individuality. There is also an expectation by the NMC that nurses will understand the legal frameworks involved with advance/anticipatorycare planning to ensure their practice is lawful. Furthermore, as district nurses provide end of life care to individuals who choose to die at home, their engagement and commitment, to not only understanding, but promoting the importance of these processes is vital. This will ensure that wishes regarding treatment and care for patients receiving anticipatory and end of life care is not delayed, and that their feelings, beliefs and values are respected and understood (Griffith, 2006).
