Learning from deaths


The death of Connor Sparrowhawk AKA Laughing Boy shook the team at @WeLDNurses to our core. We knew we had to take action and we were fortunate enough to be able to work alongside the #justiceforLB campaign and #107DaysofAction during which we hosted three chats to highlight some of our concerns and how we think that learning disability nurses can support the reduction in premature deaths.


We know that people with learning disabilities have been dying prematurely for decades, the Confidential Inquiry into Premature Deaths of People with Learning Disabilities (CIPOLD) identified that on average people with learning disabilities die sixteen years sooner than people in the general population, this isn't new news, but in the 21st century nursing landscape is it being properly addressed? Is there enough importance attached to it?


In 2016 the CQC published their report entitled Learning, Candour and Accountability, and a friend of @WeLDNurses - @GeorgeJulian stated we need to stop talking about learning lessons, and to actually make change happen. So my first question is what have we done to affect change?


The CIPOLD report had specific themes within it and one of particular interest to me in acute services is the use of Reasonable Adjustments, how we identify them and then instigate them can significantly alter the patient journey - and the family/carer experience too.


Reasonable adjustments need to run through every care pathway in acute services, where I work there are a number of these which have been embedded in clinical practice. We have several learning disabled patients who experience pseudo obstruction, a painful abdominal complaint which necessitates the use of a tube to deflate the bowel and this had resulted in numerous inpatient admissions. Not any more - on admission to the Emergency Department (ED) the surgical consultant is informed and the procedure is delivered in ED enabling the patients to return home within four hours. This is something I feel hugely proud of, as it was the surgical team who implemented this initiative without any prompting from the learning disability team.

When I recently saw the #learningfromdeaths hashtag I knew the time was right to revisit the CIPOLD and the #justiceforLB work. We know from the CQC review and #justiceforLB that families are often excluded from decisions about clinical care, and are not being fully involved in investigations following deaths. @GeorgeJulian is currently on a trip to Australia and new Zealand to see how families are involved in death investigations there #WCMTLD - have a look for this hashtag. In Australia people with learning disabilities have similar rates of premature mortality to the UK.


What are we doing as Learning Disability nurses to ensure that learning disabled people, and their families are included fully within their care during their lifetime, and any investigation following their death? What improvements to patient safety or access to healthcare have we made as a result of people's deaths?

First of all, team @WeLDNurses would like to extend our warmest thanks to @George Julian for supporting this chat and making it happen, and for engaging with our colleagues in Australia and New Zealand to make this learning from deaths chat truly international. We feel it is of vital importance that we learn from deaths, share that learning, adapt our practice to embed that learning and improve people's experiences of healthcare services.

Our second big point is that we believe that if healthcare staff can get it right for people with learning disabilties they will get it right for everyone.

The first part of the chat we discussed the reasonable adjustments, what we are doing to make reasonable adjustments and what the barriers are in practice to ensuring reasonable adjustments are made in a timely way. There was the usual list of potential reasonable adjustments, such as desensitisation visits, hospital passports, liaison with primary and secondary care to plan for admissions, accessible information and many more. It is really encouraging that reasonable adjustments like these are being discussed openly with general healthcare providers, but we learned during the chat that there are still barriers to delviering reasonable adjustments.

One of the main barriers discussed was that of staff attitudes, with some general staff feeling the reasonable adjustments are too time consuming and this can require some careful negotiation from the LDN to change those values or at least get the staff to try out working through reasonable adjustments. Often, once general staff have tried the reasonable adjustment pathway and seen how beneficial they can be for patient, family and staff alike, they are often more likely to be on board when they meet their next learning disabled patient.

One of the points echoed by several people on the chat was the need for ongoing education and dedicated acute liaison nurse roles within acute healthcare providers. There were some concerns that in acute services hospital passports still are not fully embedded, and get referred to only when there is a crisis - we feel that ongoing education and support from acute liaison nurses would address some of this.

It was sad but reassuring to learn that people with learning disabilities in New Zealand have similar services and experiences to people in the Uk, and the learning disabled community is still in part isolated from other groups in society.

When we started to talk about the really tough topics, what happens when people with LD die and who supports family and friends, we were again interested to learn that in parts of New Zealand there are only reviews of deaths in socially provided accommodation. Although the dealth reviews have been happening in the UK for some time now, again this is patchy and some areas are better developed and engaged with this process than other areas.

All deaths of people with learning disabilities are now reported and recorded, so we have some more robust data in terms of numbers and causes of death and this must be considered when planning training and education for learning disability services.

One of the concerns highlighted was the ability to support family and paid carers after someone has died. Experiences again were varied, but one theme was that caseload pressures often resulted in LDNs not feeling able to support families in the way they wanted to, and paid carers got even less support. One LDN on the chat was engaging with their local hospice for clinical supervision, and this would be s useful conduit to improve access to and information sharing with hospice services. We LDNs need to engage fully with end of life care.

We also need to acknowledge that some paid carers will have supported the same learning disabled person for many years, and they will experience grief at the death of the person they have cared for. We need to include paid carers in our networks of support.

Overall we felt that this chat provoked some really good conversation about learning from deaths, engaging with families and supporting the education of general medical staff. However there is still so much to do to ensure we reduce the rate of premature death, we really do listen to family carers and that we are trained and confident to provide emotional support for families and paid carers in times of grief.
