Nursing and Ethics in the Genomic Era

A sixty-five-year-old man has been diagnosed with a genetic condition that could have serious implications for his children - but he doesn't want them to know. Who has the right to decide?

Genomics, the study of the all the DNA in our cells, is a powerful tool that is revolutionising healthcare as we know it. The information stored in our DNA can help inform diagnosis and management - in many cases allowing us to tailor healthcare to an individual and provide truly personalised care. But, as powerful a tool as genomics can be, there are major and sometimes very specific ethical implications and considerations that arise from this kind of testing; considerations that we, as health professionals, must be equipped to deal with as genomics moves out of the specialist clinic and into the mainstream.

Why does genomics differ?

The results from a genomic test, unlike an x-ray or MRI, can not only reveal information about a person’s current health, but also, for example, about the likelihood of them developing a condition in the future, or how they will react to a particular medicine. In addition, as our DNA is inherited from our parents and passed on to our children, results showing changes in DNA often impact family members. Genomic testing can also reveal susceptibility to a condition or conditions that were not the primary reason for investigation.

What does this mean for nurses and midwives?

Genomics is increasingly encountered in routine practice. Nurses and midwives have more contact with patients than anyone else and need to be aware of the misconceptions and ethical issues that arise so that they can confidently explain to, address the concerns of, and reassure families in their care.

During this chat we would like to explore the ethical considerations and questions surrounding genomic testing that set it aside from other medical investigations. Are they really that different? What have your experiences been as a nurse or midwife?

On Tuesday 6th March we will be broadcasting a podcast covering Ethics in genomics, before the twitter chat please listen to this podcast featuring Anneke Lucassen Professor of Clinical Genetics at the University of Southampton who describes some of the challenges raised by genomic medicine.

    

Questions will include:

Do you know what genomics in healthcare is?

Can you think about any ethical questions that might arise as part of these processes, or have you encountered any in the past?

How confident would you feel in dealing with these issues?

What role do you think nurses and midwives play in dealing with ethical issues and questions?

What concerns you about dealing with genomics as a nurse or midwife?

Do you think genomics will change your practice in the future?

     

What is the Genomics Education Programme?

The programme was founded to ensure that healthcare professionals in the NHS are equipped with the right skills and knowledge to deliver a world-leading genomic medicine service. It has three main aims:

1) Support the wider transformation of services to integrate genomic technologies into healthcare.

2) Up skill existing staff so they can make the most of genomic technologies in their work.

3) Support NHS professionals involved in the 100,000 Genomes Project.

Michelle Bishop

Dr Michelle Bishop is the Education Development Specialist for HEE’s Genomics Education Programme. She studied a PhD in the field of genetics education and has since specialised in genetics and genomics education for Health Education England. She also previously worked as a genetic counsellor. Michelle provides educational guidance and contributes to the development of genomics-related resources and curricula for the specialist workforce, as well as driving wider NHS workforce transformation.

Ed Miller

Dr Ed Miller is Senior Education Development Officer for HEE’s Genomics Education Programme. Ed provides scientific and educational guidance for the development and production of a range of resources for the NHS workforce. Before joining the GEP, Ed completed a PhD in DNA damage repair and replication at the University of Birmingham. During his time as a researcher he gained valuable experience in science communication and public engagement by planning and presenting at a number of educational events for a wide range of audience

@WeNurses Lots specific to my practice inc pts withholding family info so difficult to alert at risk relatives, parents refusing to get children tested, non paternity issues etc. but wider issues can be quite different #wenurses
