Healthcare in the UK continues to change with all sectors (statutory, 3rd & independent) moving toward more commercial models. AHPs are exposed increasingly frequently to conversations about cost and value, asked to think entrepreneurially, become involved in tender processes, performance and contract monitoring.

We have a responsibility to understand and not fear talking about cost and value – the way money is used is important and health spending impacts on all our lives. The NHS and social care sector is spending public money, health insurance schemes are spending and impacting on premiums, occupational health schemes have financial impact on employees and customers, third sector organisations are entrusted to spend wisely by their donors, individuals who pay directly for healthcare rightly expect quality and value.

This tweetchat will take the changes toward more commerciality (across all sectors) as a given and seeks to explore how AHPs should respond, what support is needed, how to adapt to consumers and customers – who may or may not be the patient.

Make business your business and join in the conversation! Here are some questions to think about before the chat:

About our guest hosts:

Vicki Gilman MSc MCSP, Neurovation Ltd (@VickiGilman): I qualified as a physiotherapist in 1988 and worked through a range of specialisms in the NHS before finding my clinical niche in the field of neurorehabilitation.

Somewhat by accident I found myself in the independent sector and discovered an interest in entrepreneurial activities alongside my clinical professional background. For the past 18 years I have been able to combine both aspects as the founder of two successful and award-winning healthcare businesses and to experience exits from both. 

I learned from the ground up how to balance innovation and passion for effective, meaningful services with a well-developed attention to detail, a strong strategic view and sustainability of outcomes for both the individual with healthcare needs and for the business. I have had the good fortune in business to be surrounded by teams of outstanding people where 2-way knowledge sharing was firmly embedded.

I believe that more strong, successful healthcare businesses are needed in the UK and that health professionals and people with healthcare needs should be at the forefront of developments. I now work to support the development of health entrepreneurs and healthcare businesses across all sectors.

Chris Martey (@ChrisMartey): I qualified as a physiotherapist in February 2015 and currently work as a Band 5 Rotational Physiotherapist within the NHS. I developed a particular interest in service improvement throughout my postgraduate studies and now want to gain a better understanding of the changing dynamics of healthcare in the UK.

As a student I was keen to gain an insight into the business of healthcare, including: NHS commissioning, public spending on healthcare services and how Physiotherapists can evaluate their services in comparison to others.

Recently, as chair of the Chartered Society of Physiotherapy’s Student Representatives in the South East, I led a group of enthusiastic and innovative students in discussions of therapy-related business and business concepts which we termed Physness. Through discussions of Physness it was found that more often than not students were uninformed about business concepts and how they might be relevant to their future careers in Physiotherapy.

I feel that by encouraging discussions of business and the sharing of ideas, students, physiotherapists, and other allied health professionals will benefit from an enhanced view of the landscape of healthcare in the UK.

It was a busy chat, with lots of contributions and further idesa for future chats. Here's a summary from @VickiGilman:

Q1. Do you know how your service is funded - how can you use information on service funding & cost of interventions? 

There was a mix of AHPs who knew how their service was funded and those who did not and a feeling that access to the information may depend on seniority in some organisations. The utility of information for on cost AHPs was broad and included;• justifying cost of treatment 

• savings over alternatives,
• service planning
• localism
• developing AHP understanding of best use of resources,
• understanding efficiency savings
• Setting objectives –for the service and individuals


Many contributors shared helpful information including;

• Bundled tariff costs can be unbundled 
• Knowing about service costs has important links to 5YFV and policies in other regions of the UK
• NHS finance staff are helpful and happy to be approached
• Local health maps can help (see pre-tweetchat information)
• Some AHP training includes information on cost and how to use it
• Awareness of cost can be greater outside the NHS – eg social enterprise & private sector
• PbR (payment by results) information is available from the Department of Health, for example this simple guide 
• AHPs should aim to understand the terms ROI (return on investment), VFM (value for money), FAB
• @HFMA_UK has FREE education resources on healthcare finance 

Q2 How is value measured in your organisation? 

The use of PROMs seemed widespread, in some areas activity rather than outcomes may be the focus for data collection. Experiences of a range of other ways to measure value/KPIs were shared, including ‘covert’ evaluation and top tips for saving money.
It was suggested that value may differ depending on the ‘audience’ and that AHPs need a ‘versatile pitch’ to be able to demonstrate value. There was general agreement that there was no single way to demonstrate value across AHPs but that showing the impact of a lack of AHP input could be effective alongside the positive effects. There was a proposal that being valued by client means increased value for the organisation. The link was made between cost and value, with the question; are we looking at value if we don't know our costs?


Q3. What transferable skills do you have for business? 

The overall theme in answer to this question was that AHP’s are ‘experts in working with people’ and that ‘business is all about working with people’. The adjectives were many and varied, including; persuasion, motivation, communication, selling, creative, innovative, adaptable, patient centred, passion, dedication, problem solving, goal setting, integrity,flexibility, engagement,resilience, persistence, humour, rapport building, time management, prioritisation, proactiveness, logic,reasoning, team players, reflective, understand people.

A contributor posed the question ‘…are they enough?’ and in addition some felt that aspects of financial literacy, stakeholder management and business planning were important. 

Knowing how to apply transferable skills to business was discussed and some felt that ‘business’ was somewhat challenging and even daunting and taboo for healthcare professions, whilst others asserted that ‘business’ is a wide concept and equally relevant and empowering across all sectors, including the NHS. There was discussion around those with financial responsibility being a part of the overall healthcare team. 

Q4 What would build greater commercial confidence for AHPs? 

This first response to this question was short. It said – “ Practice. Got to dive right in and start walking the walk”

Others suggested AHPs need courage, relationship building and education, including in pre-reg curricula as a rule not an exception. Understanding the role of finance, corporate aspects and balancing business with public good was felt to be important.
There was a view that business thinking is less appealing in the public sector – and that this led to missed opportunities , thus opportunities to build business confidence are also missed. Conversely the idea that entrenched underfunding has led many to be entrepreneurial and thus forced innovation to occur was posited.

As discussion developed #SHCR – School for Health and Care Radicals was mentioned with the idea that ‘AHPs need to rock the boat without falling out of it’. The 5YFV was highlighted as requiring skills to be built and information was shared on potential benefits of getting alongside ‘valuemakers’ working in NHS finance departments.

Contributors cited success using "buzz words" raided from the strategic / business plan, learning from getting alongside finance colleagues and it was pointed out that AHP;s are bright people with the ability to learn the skills required. Role models were highlighted.
