Smoking is not a lifestyle choice but a chronic relapsing condition needing treatment. This is recognised by the government's commitment for the NHS in England to become smokefree. Supporting smokers in contact with the healthcare system to quit is a prevention priority in the NHS Long Term Plan and every health and care professional has a role to play.

    
As a healthcare professional, you are best placed to motivate and support smokers on their quit journey. Smokers expect healthcare professionals to ask about their smoking.

   

All healthcare professionals should identify and refer smokers using the method known as Very Brief Advice (VBA) which has 3 components - 'Ask, Advise and Act':

   
1.      Ask and record smoking status: is the patient a smoker, ex-smoker or non-smoker?
2.      Advise on the best way of quitting: the best way of stopping smoking is with a combination of stop smoking aids and specialist support.
3.      Act on patient response: build confidence, give information, refer and prescribe.
    
Different approaches suit different people so if the first thing a smoker tries does not help, they should try another way. We've published guidance HERE to support you in your conversations with patients on what method to choose.

All health and care professionals should be:

   
*         aware of latest guidance and interventions on supporting people to stop smoking
*         able to access learning and development opportunities to support smokers to quit
*         able to support smokers to access local referral and care pathways to stop smoking support
*         able to provide feedback to local commissioners and providers where services are working well and where there are problems accessing support for smokers who want to quit.

Smoking and tobacco can be quite an emotive topic, especially in social media spaces.  Our top tips for engaging in sensitive topics in social media are:

  

  • Think before you post
  • Respect other people's perspectives
  • It's ok to disagree but do so professionally
  • Remember social media is a conversation and no post / comment should be taken in isolation
  • Ask for clarification / context to someone's post if needed 
  • Remember just because social media is instant this doesn't mean you have to be

@WeLearnOutLoud #AllOurHealth it is an important to ask smoking status as a community OT. Can lead to all sorts of further questions- access, safety smoking, memory of techniques, fire risks, smoking in bed, compatibility with equipment. Potentially can give advice to every smoker

@WeLearnOutLoud @WeNurses A smoker is more likely to develop certain conditions so this allows the HCP to inform the patient of this and can raise awareness of the detrimental affects it has on the body and the range of treatments and support the person can have to encourage them to give up. #AllOurHealth

@WeLearnOutLoud I recently saw a bottle with the rare children get from second hand smoke and from expectant mothers smoking. It was nearly a full bottle. Sometimes you aren't just trying to support one person but two. #AllOurHealth

I think VBA is really important because to much information will overwhelm the person possibly resulting in carrying on and no intention to stop I think it’s best to briefly explain the impacts of smoking on their health and recovery #AllOurHealth

#allourhealth I used to run a smoking cessation clinic in the early 2000's and it's nice to see the numbers of smokers dropping. However, there's still a lot to do! It has been interesting to have a refresh of knowledge. It is always worth asking the question...

@NaomiBroadhead @WeLearnOutLoud you could be exposed to second hand smoke though which is just as deadly! The bottle method is ace as our lungs are out of site and out of mind so we cant see the detrimental impacts its having. #AllOurHealth

@WeLearnOutLoud keeping up to date on all treatments and info about smoking will be helpful. But I think having the confidence and being able to know when to step away from someone who doesnt want to give up knowing you have offered everything you have will be the challenge. #allourhealth

I have used the NCSCT free training module on VBA and gained an insight on how to use VBA in practice. We need to move away from the old fashioned approach which is to view addicts by our own moral codes, rather than treating addiction, rather chemical dependency as a medical condition. This approach is failing smokers, alcoholics and other addicts #AllOurHealth

@CarolineKin @WeNurses there are many people who have never suffered with CVD or Obesity who work to stop people who suffer in those areas. Overcoming an addiction is never easy and I believe that, if nessecary, it can be better for your longterm health to use e-cigs or patches for #AllOurHealth (2/2)
