Documentation takes up a large proportion of nurses’ clinical time and producing detailed documentation in line with the Nursing and Midwifery Council’s guidance is challenging. The volume of clinical documentation has grown over time as a result of:

  • Nurses taking on many of the roles that used to be undertaken exclusively by doctors; these require specific documentation
  • Complicated conditions and therapies requiring the support of multi-disciplinary teams across complex care pathways need to be documented and communicated
  • Worries about litigation require nurses to record why they took particular actions rather than just describing them
  • The ability to keep people alive despite major life-threatening conditions means many long, complicated medical histories and complex needs – all of which need to be documented and communicated to the multidisciplinary teams
  • The change in medical and nursing culture, whereby patients/parents/carers are now seen as partners in care, involves more communication than used to be required in a paternalistic medical environment

@WeNurses I find it important especially in terms of reviewing care plans.....gives you time to reflect on the care given and actually show you know your patients (or at least for me in a care home setting). I find it frustrating to see "no changes" what does that even mean????? #wenurses

@AgencyNurse @WeNurses Yeah, I really get that... I once heard a @TEDTalks that touched on a patient that was cared for in a MH Hospital. She basically said that while her care was documented, her voice wasn't noted, valued or recognised, she was just "described". Very sad! #WeNurses

@AgencyNurse @WeNurses @TEDTalks We've just had some laptops delivered after merging with a new trust, so I am hopeful we can get better at using this. Personally, I have started doing care plans on word documents, so the patient can have their say, check pronouns, etc; before adding it to the record. #WeNurses

@WeNurses I write notes that are too long. Need to learn to make them more concise so that they are more accessible and to save myself time but I'm still learning what information is necessary and what isn't. #WeNurses

@WeNurses #wenurses As a learning disability nurse I’d like to see documentation that is accessible for our clients. Accessible care plans / letters are essential.

@WeNurses #wenurses having documented information, having the permission to share information across organisations & systems can be a barrier. Also documents that don’t reflect the needs of the clients we work with. Fitting people into systems. No flexibility to create better docs.

@WeNurses #WeNurses Training & support for staff to become more confident with documenting / using IT, etc. Availability of good IT support to iron out glitches quickly. To have the right IT equipment to do the job.

@WeNurses #wenurses Better use of the Summary Care Record to flag specific requirements / reasonable adjustments client may need. Better info sharing agreements between organisations / agencies.

@AgencyNurse A4 Was on a fab webinar by @EPIC_eHealth that was not only looking at speech to text software that some of my clients use already) but using BOTS with it to in order to help documentation by speech and using algorithms to speed it up - commonly used phrases etc #WeNurses

@WeNurses #wenurses I think I’d rather spend time with the client but acknowledge the need for documentation. Having clarity re: what MUST be documented & a process in place to follow may make it easier.
