This International Nurses Day 2021, we are keen to bring together two global challenges for Nurses working with people with learning / intellectual disabilities. 

Global evidence continues to present a picture where the health inequities experienced by people with learning/intellectual disabilities has a significant impact on health outcomes. In the UK, government reports indicate that people who have a learning disability are up to six times more likely to die from COVID-19 than the general population (Johnson, 2021). A cross sectional study of more than 60 million patients from 547 US health organizations revealed that having an intellectual disability was the strongest independent risk factor for having a COVID-19 diagnosis, and the strongest risk factor aside from age for COVID-19 mortality (Gleason et al., 2021). 

The World Health Organisation recent publication seeks to strengthen Nursing and Midwifery across the world over the next four years in four specific areas:-

  •  Education, 
  • Jobs, 
  • Practice and 
  • Leadership (Iro & McCarthy, 2020)
  • This focus provides an opportune moment for Nurses working with people with learning/intellectual disabilities to work together on an international approach to mitigate these health inequities. 

    One such research project that is reaching out to Nurses across the world is run by US-based Developmental Disabilities Nurses Association to identify challenges nurses face in meeting the care needs of people with learning/intellectual disability and other developmental disabilities during the COVID-19 pandemic (Desroches et al., 2021). 

    Results revealed numerous challenges ranging from the individual level to the policy level. Highlights of these findings include: 

  • Individual: Nurses' feelings of fear, anxiety, burnout, guilt, as if they're not doing enough. 
  • Interpersonal: Helping people with learning disabilities and their support staff understand and cope with pandemic-related changes; shift from face-to-face communication to telecommunication.
  • Organizational: Exclusion of LD nurses from agency and public health planning and policymaking; association of LD nursing with medicalization of care
  • Community: Closure of health care offices and/or refusal to see people with learning disabilities for face-to-face visits; lowered standards of care 
  • Policy: Multiple, inconsistent health policy guidelines that do not reflect settings where people with learning disability live and spend their time
  • One overall key theme that emerged is the underutilization and lack of recognition of the importance of the Learning / Intellectual Disability nurse to promote and protect the health of people with learning disabilities.

    @WeLDnurses Having much needed services delayed seeing a resident and when they were seen it was a telephone consultation which was very hard to show their mental health decline. Had to real fight to get a face to face #WeLDNs

    @DrMDesrochesRN Another thing many of the people I support who have autism diagnosis have really struggled with the grey areas, things being open but not, or you being able to do some shops but not all, really difficult for there to be such strange rules in place #weldns

    #weldns It sounds like mental health and support continue to be a major concern for people with LD and LDNs and LDN students alike. I think COVID has really exposed existing challenges related to mental health and well being for people with LD and nurses!

The population of people with LD in the US are referred to as "invisible" in our data, and hence our health policies. Even conducting research is difficult without data.

    @WeLDnurses I think more than ever we have seen both the importance & benefit of the holistic, biopsychsocial approach we take to supporting people & how building a rapport & knowledge of the person helps this through this pandemic,I want us to keep shouting this to the world! #weldns #IND21
