There has been a trend in increasing numbers of under 5’s being admitted to hospital with mild conditions for less than 24 hours putting added pressure on already overstretched services. Many of these could potentially be managed at home with the right support and advice (Wessex Healthier Together Website).

During this chat we will look at some of the most common admissions including Gastroenteritis, Urinary Tract Infections, Tonsillitis and Chicken Pox, how these are currently managed across trusts, how we could or already are preventing or reducing the length of admissions and what advice we currently give to parents and families to be able to manage these at home.We welcome you all to join us in this chat to share your thoughts, ideas and best practice

After welcoming people to the chat, our first question of the evening was ‘UTIs – how do you currently manage presentation of these within your trust?’

@cdodd09 mentioned that getting a fresh urine sample can be difficult to obtain and@spencer_sian added that water play can be used to help obtain clean catch urine samples. Other contributors also mentioned the use of urine bags, cotton wool and urine pads in the collection of urine samples and the pros and cons of these methods.

Question 2 looked at how we could prevent/reduce the length of admissions for UTIs and do you already have pathways in place to do this - @hardcastle76,@Csage110 and various other contributors discussed the role of Community Children’s Nurses (CCN) working in GP surgeries for early identification and testing prior to admissions. This led on to alengthy discussion in relation to the use of CCNs or paediatric nurses within GP settings and issues around numbers of CCNs, funding,differences in how these teams work and CCN qualifications as these seem to vary across the country.

We then discussed what are the best methods for giving advice to families - @SarahNeil17stated the importance of giving parents reliable information as the internet is relied upon by them and other contributors also discussed how families rely heavily on the internet with information however they do not know what are reliable sources so we as professionals need to signpost them to the best resources.

The final question was‘in an ideal world what would be the one thing you’d like to see happen in helping to reduce admissions, @spencer_sian said that she’d like to see a wrap around CYP service where we are all working together. Other suggestions included better parent education,positive informative interactions between parents and professionals,better community support and a fully commissioned CCN service
