It is now common knowledge that because of the advances in medicine in technology, people are living longer with more complex conditions. Therefore effective skill mix is essential for the nursing profession, to move forward, to meet the competing demands, whilst also promoting patient safety and quality care for a better patient experience.

Community nursing services are delivered by a variety of disciplines including, district nurses, staff nurses, nurse practitioners, matrons and health care assistants, to name a few. However, what we need to ensure is that the right nurse with the right skills, at the right time is delivering the care to optimise patient safety and the patient experience as highlighted by Jane Cummings (CNO) (NHS Commissioning Board,2012).

There are further challenges managing community nursing services. As the QNI (2011) identify, it is now well established that care is shifting from hospitals to the community setting but the number of District Nurses reportedly has decreased by 40%. This raises concerns around dilution of the service as well as fears around recruitment and retention. Additionally the number of healthcare assistants carrying out tasks, which would have previously been done by qualified nurses has doubled.

Recent public enquiries into health care services, whilst mainly focusing on the acute sector, raise issues for consideration within community nursing. Sir Bruce Keogh (2013) suggested an over reliance upon unregistered staff in delivering care, The Cavendish Review (2013) also identified that over 1.3 million frontline staff, who are unregistered, are delivering the bulk of hands on care. The Francis Inquiry (2012) highlighted the need to re-build trust and confidence in the services provided. Unacceptable individual or organisational failings must be addressed and the public have the right to receive quality, safe and effective person centred care. Therefore effective leadership, by everyone, to promote effective skill mix, promoting a positive organisational culture whereby staff feel supported and valued, is required.

In recent years, newly qualified staff are coming straight to the community setting, therefore community nursing services need to ensure that support is available to help with the novice to expert journey. Again this requires investing time to nurture, to learn/coach and support staff, which requires investment in education, training and workforce.

Community nursing services must strive for excellence and ensure that all staff regardless of role are delivering high quality, safe and compassionate care to patients and their relatives/carers within their own homes.

  • What leadership qualities are required to promote the right nurse, right, skill at right time aim?

I have been informed by some in the UK NHS health service profession, that I am an an experienced patient . Not sure if I should accept this title with humility or accept a mythological golden crown of honour.But now this but should I possibly state "That I believe in and trust you boys & girls in the NHS" It is sad that most politicians & marketeers choose not to accept or believe in my personal judgement & experience & choose to make choices upon economic decisions . Many of which I have personally contributed to for over 50 years ! Is it that I have been conned by successive Governments for half a century. Now it appears you are also to become included in the fraud pretence of liars & psychopaths ?
