NICE published an update of the guidelines for Intrapartum Care in December 2014 #EBP. I was a member of the guideline group who developed these. The guideline has many changes to practice within it. I think the most exciting yet challenging part of the guideline are the recommendations for place of birth, and this is what I would like this chat to focus on.
When I performed my research for my PhD on Women and Midwives Perceptions of a Midwife's Role it seemed clear that many women feel the safest place for them is in hospital because doctors are there and technology is available (Cooper 2011).
The results from studies on place of birth blow this perception out of the water, clearly showing birth is safer at home for women who have already given birth to a baby and a freestanding birth centre is the safest place for women having their first baby. See tables below outcomes for women and their planned place of birth, the yellow shaded areas show the best outcomes for these women.
Women who have had a previous baby:
Babies have the same outcomes in all of the settings 2-3/1000.
Women having their first baby:
Babies have a slightly increased risk of an adverse outcome for women planning birth at home 9/1000, compared to 5/1000 in the other settings.
These tables are included in information about place of birth in the implementation tools for midwives and doctors to discuss with women, do you think this will happen?
I think that not only women but midwives and doctors perceptions relating to the safest place for birth are challenged by this evidence. In view of this how successful do you think this part of the guideline will be? Clinical Commissioning Groups have to ensure all 4 birth settings are available, including freestanding birth centres. Do you think this will happen? For women to make a truly informed choice, are they directed to the guidance in your service, which clearly highlights disadvantages of birth in an obstetric unit for low risk women?
There has been criticism of the studies included, but one of these studies is the largest study performed on place of birth in the UK, therefore is based on current services within the UK, providing the most up to date information on almost 75,000 births (Birthplace 2012). Is this because many professionals do not feel comfortable about what the evidence suggests?
