Social prescribing is about helping people to live the best life they can. A life that is defined by them. Not one that is limited by the place in which they live, who they are, or the resources available to them. Social prescribing means supporting people, via social prescribing link workers, to make community connections and discover new opportunities, building on individual strengths and preferences, to improve health and wellbeing.
It is about understanding that people’s health is not just a medical matter. Isolation, struggles with mental health or living with various long term conditions can all make it difficult for people to get on with their lives. People will always be able to get the medical care and treatment they need but on its own it’s not always enough. So NHS England and NHS Improvement have committed to funding social prescribing link workers in every primary care network across England.
Link workers are key to social prescribing. But so are the GPs and PCN teams who refer into them. And so are the voluntary and community groups who provide the range of activities and support that people can be linked into. And so are the commissioners, local authorities and others who can help organisations to join up their plans for social prescribing.

@WeNurses Without a doubt, education on its benefits to all health care professionals and stakeholders (including patients!) #WeMDT , and also ensuring that all professionals are encouraged to sociallly prescribe. It shouldn’t be down to just one individual! |
