Hosted by WeNurses using #AllOurHealth

Tweetchat times will be split into three groups organised by birthday:

Group A - January to April Birthdays take place from 6:00pm - 6:30pm

Group B - May to August Birthdays take place from 6:45pm - 7:15pm

Group C - September to December Birthdays take place from 7:30pm - 8:00pm

In England nearly 7 million people are living with cardiovascular disease (CVD), it contributes to over a quarter of deaths each year and health and social care costs in England relating to CVD are estimated at £7.4 billion each year. (PHE 2020)

CVD remains an important issue as it’s a contributor to mortality, morbidity and health inequalities and stalling of reductions in CVD mortality. This is costly to the individual, the NHS and wider system.

The infographic below identifies some of the risk factors for CVD:

(PHE 2020)

To achieve a change in individual risk factors and an individual’s overall risk of CVD we need action which will enable behaviour change at population, community and an individual level.

The behaviour change wheel (below) published by the University College London helps us to understand the types of policies and interventions that have an influence on behaviour.

(Michie 2012 )

This tweetchat aims to explore CVD, behaviour change and personalised care and what we, as health care professionals can do in our day to day roles to prevent CVD. The questions we will be asking in the Tweetchat are:

  • What sort of behaviour change do people need to make to reduce the risk of CVD?
  • How can personalisation help us as healthcare professionals to address cardiovascular disease in our day to day roles?
  • What practical action can you put into practice immediately to reduce the risk of CVD for the people you care for?

 Chat Summary

Another set of amazingly informed and passionate learning out loud from the students from @HullNursing who have now on the whole mastered the art of tweetchatting, learning out loud, raising and discussing ideas and looking out how these can be put into practice. If you missed the chat here is a summary of some of the questions and responses from the students.

We start our summary by introducing an extremely passionate, experienced tweeting nurse who will always be on hand to direct you to support for CVD on Twitter Prof Jamie Waterall, do give him a follow:

Moving on to the chat we started with the following question, lifestyle was a common theme the students picked up on:

We moved on to looking at how an individual is supported to reduce CVD risk, the students understood that all patients are unique and how they are approached, advised and supported needs a degree of personalisation too:

Lots of learning and evidence was shared on the chat both in discussion and in responses to the question... BUT... how do we put this learning into action. Whilst the students may not be practicing at the moment we drove them to consider how they would action this knowledge, here are some of their thoughts... how many practicing nurses do these things on a regular basis...

We thought we would finish the summary by sharing this tweet - great to see this style of learning being something the students are enjoying!

Love your heart, love tweetchatting, love supporting students learning out loud on Twitter!

RT @VickyCoxon6: @WeLearnOutLoud Stopping or cutting smoking, healthier diet and more exercise If possible #AllOurhealth
@stndemi
23 February 2021 18:03
@WeLearnOutLoud Look at their own lifestyle factors, do they drink? Smoke? How much physical exercise do they get? Are they healthy? All of these things can reduce the risk factors #AllOurHealth
@Savanna78205845
23 February 2021 18:03
In order to reduce the risk of CVD you have to adopt willingness and motivation, you have to want to change your lifestyle in order to have a healthier life. Healthy life, healthy heart! #AllOurHealth https://twitter.com/WeLearnOutLoud/status/1364274072562065412
@ElianaMann19
23 February 2021 18:03
@WeLearnOutLoud In order to reduce risks of CVD it is recommended to engage in daily exercise. In these times, an outdoor walk is recommended or even an outdoor gym can be useful! #AllOurHealth
@SabeloS72245672

@WeLearnOutLoud Changing life style can be a good starting point #AllOurHealth
@Deborah21172841
23 February 2021 18:04
Smoking cessation, lifestyle and diet changes, including reducing salt intake can all contribute to reduced CVD risk. Awareness of contributory factors is key to combatting this disease #AllOurHealth
@WeLearnOutLoud
23 February 2021 18:04
@ElianaMann19 @Savanna78205845 @stndemi @VickyCoxon6 great start to tonights #Allourhealth chat everyone :)
@CuttingMoira
23 February 2021 18:04
@WeLearnOutLoud It can be determined from an individual's own lifestyle factors, which include drinking (in excess), smoking, diet and exercise levels #AllOurHealth
@CuttingMoira
23 February 2021 18:07
@WeLearnOutLoud As well, environmental factors such as employment levels can all contribute towards the development of CVD. If unemloyed/poorely paid, they cannot fund a healthy diet as easy as those with higher wages and the opportunity to make healthier choices for themselves #AllOurHealth
@Jobee4t
23 February 2021 18:07
@WeLearnOutLoud #AllOurHealth We should encourage and empower people to try and reduce their risk factors such as stopping smoking, increasing physical activity and eating healthier
@WeLearnOutLoud
23 February 2021 18:10
QUESTION 2 How can personalisation help us as healthcare professionals to address cardiovascular disease in our day to day roles? #AllOurHealth Group A https://t.co/SigL3cOksC
@Jobee4t
23 February 2021 18:10
@CuttingMoira @WeLearnOutLoud #AllOurHealth Yes I agree Moira the environmental factors can also increase the life choices which place people in higher risk of CVD
@VickyCoxon6
23 February 2021 18:11
@WeLearnOutLoud Practice what you preach, dont be judgemental and to support them every step of the way even through relapse. #AllOurHealth
@Kayleigh_L88
23 February 2021 18:11
@WeLearnOutLoud Lifestyle changes such as stopping smoking, reducing alcohol intake, increasing physical activity and a more balanced diet can reduce CVD #AllOurHealth
@stndemi
23 February 2021 18:13
@WeLearnOutLoud It helps because their risk factor could be one of many, so each person would need a different plan and different support. It also helps us to treat the patient as a person, and give them information to make the right decisions for them #AllOurHealth
@Kayleigh_L88
23 February 2021 18:13
@VickyCoxon6 @WeLearnOutLoud Great point about relapses, patients need our support more than ever at that point to continue their successes #AllOurHealth
@Savanna78205845
23 February 2021 18:14
@WeLearnOutLoud Personalisation is important as we as health care professionals have a big impact on the decisions patients make when it comes to their health, we have to take all of their needs and influencing factors into consideration, provide them with as much support as we can #AllOurHealth
@Deborah21172841
23 February 2021 18:14
Ensure time to get to chat to and listen to patients, to find out about the individual's background, lifestyle, and where I can be of help. MECC ensures we can embed this in our everyday interactions. And to provide information and advice on CVD. #AllOurHealth
@Jobee4t
23 February 2021 18:14
@WeLearnOutLoud #AllOurHealth We can identify the needs of the person and provide the correct information and referral to appropriate services
@WeLearnOutLoud
23 February 2021 18:14
@stndemi how easy do you think it might be in practice to do this kayleigh #Allourhealth
@CuttingMoira
23 February 2021 18:15
@WeLearnOutLoud It allows us to be efficient when caring for the individual as everyone has their own lifestyles. By catering to that individual's needs ensures that the focus stays on them as an individual and they do not feel like just another patient to be cared for #AllOurHealth
@stndemi
23 February 2021 18:16
@VickyCoxon6 @WeLearnOutLoud Absolutely, lifestyle changes aren't easy for everyone! #AllOurHealth
@Savanna78205845
23 February 2021 18:16
@Deborah21172841 I agree, taking time to talk to individuals to build trust, building trust is one of the most important things when it comes to patient care. #AllOurHealth
@Kayleigh_L88
23 February 2021 18:16
@WeLearnOutLoud It's important to treat the patients as individuals and find out what they want to achieve and how they want to make the changed to improve their health. If they identify an area that is of particular importance to them we can support them better to reduce CVD risk #AllOurHealth
@stndemi
23 February 2021 18:17
@Savanna78205845 @WeLearnOutLoud We as nurses are the most trusted profession, so you're absolutely right that we have an impact on their decisions! It's why we need to be giving them all the information so they can help us make the best care choices for them. Teamwork! #AllOurHealth

@Jobee4t
23 February 2021 18:19
@WeLearnOutLoud @stndemi #AllOurHealth Relapse is difficult but encouragement and highlighting the positives, creates a greater chance than focussing on negatives
@WeLearnOutLoud
23 February 2021 18:20
QUESTION 3 What practical action can you put into practice immediately to reduce the risk of CVD for the people you care for? #AllOurHealth Group A https://t.co/vzVVuAF10O
@SabeloS72245672
23 February 2021 18:21
@Deborah21172841 @WeLearnOutLoud By giving guidance to patients and sharing personal experiences if relevant #AllOurHealth
@Jobee4t
23 February 2021 18:22
@WeLearnOutLoud #AllOurHealth We can MECC and identify people early and provide timely interventions to reduce the exposure to CVD. We can also inform them regarding the NHS health check which they can utilise
@stndemi
23 February 2021 18:22
@WeLearnOutLoud A huge percentage of people are undiagnosed for risk factors: high blood pressure and cholesterol. We can learn more about these things through online courses to know what signs and symptoms we need to be looking out for in order to try and reduce this number. #AllOurHealth
@Savanna78205845
23 February 2021 18:24
@WeLearnOutLoud We have to use our profession to an advantage use our knowledge to give advice to patients about the improvements they can make to their lifestyle. To use the person centred approach to give advice based on the patients needs, not everyone will need the same advice #AllOurHealth
@stndemi
23 February 2021 18:24
@Jobee4t @WeLearnOutLoud I swear I mention MECC every week but it really is vital to the entire framework! If we don't sit and have those conversations, then we can't actively support and help people. #AllOurHealth
@Deborah21172841
23 February 2021 18:24
Information and awareness of the condition and how lifestyle choices are large contributory factors. Ask also about family history to identify hereditary factors. #AllOurHealth
@Jobee4t
23 February 2021 18:26

@WeLearnOutLoud In the NMC our roles as nurses and healthcare professionals is to treat everyone as an individual and never judge them for their lifestyle choice. We must encourage them to make a better choice as this will decrease their risk of having CVD and other health issues. #AllOurHealth

#AllOurHealth something i could do is share information with the people around me and a good way to do this would be through social media. By sharing articles and infographics etc, people could be prompted to do their own research to take actions that are best for them.

@ZoeLianne1
23 February 2021 19:33
@WeLearnOutLoud Behaviour changes people need to make are mostly to do with their lifestyle choices. Diet, drinking, smoking and physical activity are all aspects that increase the risks of CVD however with support people can change these and reduce their risk factors for CVD. #AllOurHealth
@WeLearnOutLoud

@WeLearnOutLoud Changes such as stopping smoking, having a healthier diet and getting physical exercise can reduce CVD but also considering people’s mental health and the reasons for their lifestyle choices is also important when providing the best support to each individual #AllOurHealth

QUESTION 2 How can personalisation help us as healthcare professionals to address cardiovascular disease in our day to day roles? #AllOurHealth Group C https://t.co/XvJEbAudDy
@JamieWaterall
23 February 2021 19:40
@WeLearnOutLoud I published this paper with colleagues looking at the use of on-line #HeartAgeTest to promote awareness of CVD risk & what action people can take to lower this #AllOurHealth https://bmjopen.bmj.com/content/10/11/e042963
@ZoeLianne1
23 February 2021 19:40
@Heather26138850 @WeLearnOutLoud I have to agree with both heather and shan here, trying to change all lifestyle choices at one time is an unrealistic goal and can in some people cause them to be unable to meet said goal. Making smaller more achievable steps means these can be met and accomplished. #AllOurHealth
@stn_seg
23 February 2021 19:41
@WeLearnOutLoud Personalisation allows opportunities to assess the risk to each *individual* patient eg through checking their BP/cholesterol or raising awareness of the factors that can influence risk. Also allows us to assess any barriers that may prevent patients making changes #AllOurHealth
@Kodwokoomson
23 February 2021 19:42
@WeLearnOutLoud If we make those behaviour changes mentioned in our life, we can talk to our patients from personal experience when advising them #AllOurHealth
@sarahan40851386
23 February 2021 19:42
@sky_prestidge @WeLearnOutLoud these are all good ways, but people may not know how or where to get advice, support to achieve these changes. #AllOurhealth
@ZoeLianne1
23 February 2021 19:43
@WeLearnOutLoud Personalisation can help us to treat each patient as an individual, it means we can tailor support to them and what they may need, it also allows us to signpost to the correct services for the individual patient rather than the diagnosis as a whole. #AllOurHealth

23 February 2021 19:45
@vickile47947769 @WeLearnOutLoud I agree Vicki, making sure to tailor the support to the individual rather than the diagnosis can help a patient feel more at ease with the changes they may need to make. #AllOurHealth
@CGadie
23 February 2021 19:45
@WeLearnOutLoud By promoting person-centred care and getting to know persons situation, for example them smoking or being overweight. Then guiding and supporting them to improve their health and lowering the chances of CVD. #AllOurHealth
@sky_prestidge
23 February 2021 19:45
@stn_seg @WeLearnOutLoud I agree it’s really important to consider other things that could be affecting their lifestyle choices in order to help them make positive changes #AllOurHealth
@ZoeLianne1
23 February 2021 19:46
@sarahan40851386 @WeLearnOutLoud Definitely Sarah. Communication is a huge part of personalisation. it then means we know what the patients want and how they feel is the best way to move forward with support. #AllOurHealth
@vickile47947769
23 February 2021 19:46
@ZoeLianne1 @WeLearnOutLoud it would also help them to stick to the changes long term if its tailored to their own needs. #Allourhealth
@sarahan40851386
23 February 2021 19:47
@Kodwokoomson @WeLearnOutLoud communicating , offering the advice, the support or simply pointing the person in the right direction to where they can receive this information. in a non judgmental way. #AllOurhealth
@stn_seg
23 February 2021 19:47
@sky_prestidge @WeLearnOutLoud Exactly. And saying “you need to change this” without providing appropriate support or considering what may be preventing the healthier behaviours isn’t going to yield the best outcomes for the patient #AllOurHealth
@WeLearnOutLoud
23 February 2021 19:47
@CGadie Good point. We don't often have much time to do that, what do we think we might be able to do in short interactions when there are other things patients are concerned about at the time? #AllOurHealth

23 February 2021 19:48
@ZoeLianne1 @WeLearnOutLoud This is a really good point Zoe. Finding the right resources to suit the individual will help them feel empowered in their own choices.#Allourhealth
@ZoeLianne1
23 February 2021 19:49
@vickile47947769 @WeLearnOutLoud I agree, if people have an input in creating their care plans there are more likely to continue with support to achieve the steps made for them and with them. #AllOurHealth
@sky_prestidge
23 February 2021 19:49
@sarahan40851386 @WeLearnOutLoud I agree looking at each person as an individual can also allow you to provide relevant information such as promoting lifestyle change and support service for them so ensuring you know as a HCP which information to provide and where is also important #AllOurHealth
@WeLearnOutLoud

@vickile47947769 @ZoeLianne1 @WeLearnOutLoud Love this idea of empowerment ???? #AllOurHealth
@stn_seg
23 February 2021 19:50
@WeLearnOutLoud Practical action that can be taken involves accessing training such as the #AllOurHealth e-learning to increase my confidence, utilising tools such the Heart Age Test, and MECC by discussing risk factors with a patient & providing support to manage them
@Heather26138850
23 February 2021 19:52

23 February 2021 19:56
@WeLearnOutLoud Practical action that I can do involves training. Including the #AllOurHealth e-learning, also communication with my peers and colleagues to gain some more education on CVD and the tools to assess patients. while also gaining confidence on providing support for patients
